University Policy

Rationale/​Purpose:

This policy outlines the vaccination disclosure requirements for UW–Madison students to support public health and mitigate risk from vaccine-preventable diseases. 

Definitions:
Student
For purposes of this policy, an individual currently enrolled in a degree-seeking program.
Scope:

This policy applies to all students at the University of Wisconsin–Madison. This policy is an independent requirement and does not supersede any other health-related policies.

Policy:

  1. Requirements

    UW–Madison students are required to provide documentation of their immunity status for the following:

    1. Measles, Mumps, Rubella (MMR)
    2. Tetanus, Diphtheria, Pertussis (Tdap)
    3. Varicella (Chicken Pox)
    4. Meningococcal (ACWY, B)
    5. Hepatitis B

  2. Acceptable Documentation

    To comply with this policy, a student must either:

    1. Document their immunity status by submitting official vaccination records or serological evidence of immunity (e.g., lab titers) to University Health Services (UHS) for verification; or
    2. Disclose they are not vaccinated by completing the Attestation of Vaccination Status. Students will not be required to provide rationale.

  3. Consequences for Noncompliance

    Students who fail to submit acceptable documentation by the applicable date communicated by UHS are subject to placement of a registration hold until the disclosure requirement has been satisfied.
