Rationale/​Purpose:

This policy defines the minimum medical, surgical, and anesthetic records that will meet the requirements as described in The Guide for the Care and Use of Laboratory Animals (2011 edition) and the Animal Welfare Act and Animal Welfare Regulations.

Policy:
  1. Medical, surgical, and anesthetic records must be maintained on all animals used in research, teaching, or outreach.
    1. Such records must be accessible at all times for animal care and use committees, veterinary staff, and regulators.
    2. The specific information required in these records is described in detail under medical records. Specific requirements for medical, surgical and postsurgical, and anesthesia monitoring records are available at this site, as well as suggested records templates.
