Rationale/​Purpose:

To define the minimum requirements for possession and use of Class 3B and Class 4 lasers and laser systems.

Definitions:
Permit Holder
Is an individual who has overall responsibilities for safe use of lasers and laser systems.
Scope:

All procurement, use, transfer, and disposal of Class 3B and Class 4 lasers.

Policy:
  1. Registration
    1. All Class 3B and Class 4 lasers must be registered with the Office of Radiation Safety (ORS)
    2. Class 1 – Class 3R lasers:
      1. Are not required to be registered
      2. If modified, the Permit Holder must notify ORS
  2. Operation
    1. The Permit Holder is responsible for providing and ensuring completion of all applicable trainings by site staff and laser operators
    2. Any individual operating a Class 3B or 4 laser must receive a Medical Clearance from Occupational Medicine and ORS
    3. Locations where Class 3B or Class 4 lasers are used must use engineering controls and administrative controls as prescribed by the UW-Madison Laser Safety Handbook
  3. Transfer and Disposal - Lasers that will no longer be used by UW-Madison entities must be approved for transfer or disposal by ORS
